CPDAS Packet Builder · Res. 2165 format

Turn your UpCredential module documents into one PRC-ready PDF — auto-filled, correctly ordered, print-and-sign.

1How many modules are you submitting?

One certificate, program of activities, and narrative report will be prepared per module.

2Your details

Filled once for the whole packet. These populate Part I of the CPDD-03 form and the opening section of every narrative report.

Written once. It appears as Section 1 of every narrative report in this packet — you never retype it.

3My Skill Gaps and Challenges

Every module you took answered something — a challenge in the role, a change in the standards, a capability worth sharpening. List those here once, in your own words. Each module then points to whichever one it addressed.

If nothing comes to mind, answer any of these instead:
  • What has changed in the standards, rules, or systems that you want to stay ahead of?
  • Which part of the role has become harder or more demanding than it used to be?
  • What do you handle well but would like to do faster, or with less rework?
  • What would you want a sharper answer for the next time a client or the board asks?
  • What is coming up in the next year that you would rather be ready for than react to?

Three to five is usually enough for a full compliance period. Each entry is dated, which is useful if the Council asks when the challenge arose.

4Your modules

Upload the Certificate and Program of Activities for each module — the title, date, and program details are read out of them automatically. You only answer what the Council actually scores.

5Professional ID Card (PIC)

Upload one scan showing the front and back (image or PDF). Placed at the very front of the packet.

6Build the packet

Final order: PICCPDD-03 Form → then per module Certificate → Program of Activities → Narrative Report. Every page is numbered and labelled with its module.

Prototype note: The CPDD-03 form and narrative reports are generated to match the official templates, then merged. After downloading, print the packet, sign the form and each narrative by hand, then submit to PRC via CPDAS. Narrative reports follow the structure required by PRC Resolution No. 2165, s. 2026. The four development goals below are placeholder wording pending release of Annex A.
